Free Athletic Physicals Being Offered in Sweetwater Saturday

Free athletic physicals will be given Saturday, May 19 at the Sweetwater
High School athletic fieldhouse. Times for the physicals will be 8-9:30
a.m. for incoming seventh and ninth grade boys and girls, and 9:30-11
a.m. for incoming 11th graders and boys and girls with major injuries.
Student athletes who are entering the seventh, ninth or 11th grade must
receive a new physical for that year.
The eighth, 10th and 12th grade athletes may not need a physical for the school year if they already have an accepted physical on file with the athletic department, but the athlete may receive a new physical if desired.
Due to school and UIL rules, student athletes cannot participate in any athletic workout, scrimmage or game for the 2012-13 athletic season until all required forms are completed and returned to the athletic department. Therefore, all paperwork must be returned before "two-a-days" begin. Prompt attention to getting all forms completed and returned will be greatly appreciated.
The medical history part of the form must be completed each year even though a new physical may not be required. Parents/guardians should read the information on the physical examination form and fill it in correctly. They should make sure that all forms are completed and signed by the parent/guardian and the athlete before the forms are returned to the athletic department.
An Acknowledgement of Rules/Parent or Guardian Permit form must be signed in case the student athlete is injured during a workout or game. This form and the physical go into the athlete's file. If the athlete has to go to the hospital because of an injury, a copy of this permit goes with the athlete to help secure medical attention for the athlete.
The school district purchases secondary injury insurance that covers the athlete during athletics only. The athlete should have some other type of personal medical insurance. The Classroom Student Insurance Plan offered by the school will not cover UIL athletics. The school policy is a co-insurance policy. At the time of an injury, a parent/guardian will receive an insurance claim form that will explain the claim procedure. A parent/guardian is responsible for all medical bills and the school insurance company is not required to take any action on any accounts if the entire requirements are not met. The Sweetwater ISD is not responsible for the payment of any medical bills.
Due to the new SISD Drug Testing Policy, the Random Drug Testing Authorization Form has been added to the athletic participation packet. It must be signed by a parent or guardian and the student athlete.
