Although police aren't saying much, a Houston newspaper released an article giving details on the case.

It starts with the death of a toddler on Dyess Air Force base that happened nearly two months ago.

Twenty two month old Tamryn Klapheke was found dead in her home of dehydration with chemical burns on her face.

Six days before that CPS officials closed an 11 month old neglect investigation involving Tamryn.

According to the Houston Chronicle, the Texas Child Protective Services supervisor never made a required final visit to the family or had someone approve the case's closure.

Today, Tamryns mother, Tiffany Klapheke remains behind bars in the Taylor County Jail arrested on three counts of injury to a child.

Their other two children were found barely alive.

The Chronicle reports, the insulin levels in the six month old girl were so low she was near death.

All of the Klapheke's children were the subject of three previous neglect investigations between April of 2010 and September of 2011 but neglect was ruled out in all three.

The case worker at the time was promoted to supervisor and after Tamryns death, she resigned.

Tamryns sisters are now in foster care and CPS has closed their investigation.
